Documents for admission

Persons entering the educational programs of the college provide the service provider with:

1. application for acceptance of documents;
2. original document on education;
3. photographs 3x4 cm in the amount of 4 pieces;
4. medical certificate form No. 075-U, approved by order of the Acting Minister of Health of the Republic of Kazakhstan dated October 30, 2020 No. ҚР DSM-175/2020 "On approval of forms of accounting documentation in the field of healthcare" (registered in the Register of State Registration of Regulatory Legal Acts under No. 21579), for disabled people of groups I and II and disabled since childhood, the conclusion of a medical and social examination in the form 031-U, approved by order of the Acting Minister of Health of the Republic of Kazakhstan dated October 30, 2020 No. ҚР ДСМ-175/2020 "On approval of forms of accounting documentation in the field of healthcare" (registered in the Register of State Registration of Regulatory Legal Acts under No. 21579);
5. identity document (for personal identification). Identification documents of the service recipient are presented personally or by legal representatives
